How to apply for your Delaware card if you:

Took your EMT class IN Delaware

Instructions and Forms:
DE EMT Application
Background Check Request Form

1.    Successfully complete the national registry exam.
2.    Complete the ATTACHED application.
3.    Get the application signed by the Training Lieutenant.
4.    Attach a copy of your EMT course completion certificate, your National Registry card and your CPR card.
5.    If you do not have a Delaware driver’s license, email a picture of yourself and a scan of your signature (these will appear on your card) to joann.didonato@state.de.us
6.    Complete the attached form in order to obtain a background check. Drop off the completed form at the front desk of Public Safety on Academy Street.
7.    Call fire school (302-739-4773) and follow up to make sure they have everything. This is important!

If you took your EMT class in ANOTHER STATE

Instructions and Forms:
DE EMT RECIPROCITY
Background Check Request Form

1.    Successfully complete the national registry exam.
2.    Complete the application on page 2 and mail it to fire school. This needs a signature from the Training Lieutenant. You also need to send fire school a copy of your NREMT card, state card and CPR card.
3.    If you do not have a Delaware driver’s license, email a picture of yourself and a scan of your signature (these will appear on your card) to joann.didonato@state.de.us
4.    Complete the top portion of the form on page 3 and send it to the EMS office of your home state so that fire school can verify that you are certified there. Most states’ EMS office contact info is listed here: http://www.nremt.org/nremt/about/emt_cand_state_offices.asp
Call them first and find out what you need to do to get them to fill this out.
5.    Complete the attached form in order to obtain a background check. Drop off the completed form at the front desk of Public Safety on Academy Street.
6.    Travel down to fire school in Dover to take a “Reciprocity Class”.  Contact the Training Lieutenant to be registered for one of these classes.
